Senior LinkAge

The state of Minnesota requires seniors to register with the Senior LinkAge Line Prior to moving into an assisted living facility in the state of Minnesota. The Senior LinkAge Line is a service provided by the Minnesota Board on Aging and Area Agencies on Aging. The Senior LinkAge Line will complete a screen over the phone (to include questions) and provide an eleven (11) digit verification code. This code will be requested by the Housing Manager at Fair Meadow Assisted living prior to or upon admission. The phone number for the Senior Linkage Line is 1-800-333-2433. They are requiring the senior who needs assisted living to contact them. They will allow family/friends to help with this contact as long as the senior who needs the assisted living is present.
