Fair Meadow Nursing Home is a 42 bed skilled nursing facility in Fertile, MN.  We pride ourselves on the small town, close knit, home environment feel it provides residents.  


 
     
 
Hospice
  • Emphasizes living the remaining days as fully as possible.
  • Provides relief from the physical, spiritual and emotional pain that often accompanies a terminal illness.
Long Term Care
RN’s, LPN’s, TMA’s and CNA’s provide excellent care to residents 24/7





Weekly Church Service
Church and bible study are provided weekly at Fair Meadow.  Pastors also visit often to meet residents’ spiritual needs.  
Home Delivered Meals
  • Hot meals delivered Monday-Friday within city limits during dinner hours. 
  • Weekend meals can be provided and delivered with Fridays dinner.

  • Rehab Nursing - twice a day, 7 days a week
  • Therapy Aids provide Nursing Rehab daily
  • Access to Omnistim “PENS” by Accelarated Care Plus.

Weekly Hair Care
Fair Meadow Nursing Home provides an on site Beauty/barber salon. Activity aids as well as licensed hair care staff from our community provide hair care weekly.
Daily Activities
Our creative Activity Aids provide daily activities both on and off facility grounds.  Some of the favorite activities include van rides, supper dates, shopping, casino trips, site seeing, bingo, small groups, and cooking.  Fair Meadow’s goal is to find fun fulfilling activities for all residents to enjoy, whether in a large group, small group, or one on one.

We have a Velvo Plus bike so those in wheel chairs can go on a ride. We have a golf cart available for rides around town. We will soon have a wheel chair swing.
Alzheimer's - Dementia
We do not have a locked unit, but we do provide memory care. All of our staff has dementia training. We are well equipped to handle the Cognitive and sensory changes that accompany dementia:
  • Memory loss, generally noticed by the near and dear ones
  • Difficulty in communication, especially finding the right words to communicate
  • Reduced ability to organize, plan, reason, or solve problems
  • Difficulty handling complex tasks
  • Confusion and disorientation
  • Difficulty with coordination and motor functions
